Small Business Health Benefits in Winchester, VA: What Low-Cost Direct Primary Care Actually Costs
If you run a small business in Winchester, the benefits of math have probably stopped making sense. At companies with 10 to 199 workers, the average family plan now costs $26,054 a year, and the employee pays $8,889 of that themselves. That is more than $2,000 above what the average worker at a large employer contributes.
Then there is the part nobody puts in the enrollment packet. The average single-coverage deductible at a small firm is $2,631. So your employee hands over close to $9,000 a year and still cannot get a sore throat looked at without paying out of pocket first.
It is not surprising that only 59% of businesses your size offer health benefits at all.
There is a cheaper way to give your team real primary care, and it does not require a group plan. Here is what low-cost direct primary care for small businesses costs and how Winchester employers are actually using it.
How Much Does Direct Primary Care Cost?
We publish our rates, so you can see a number without filling in a form. There is a one-time $100 registration fee per member, capped at $300 per family.
Membership | Monthly cost | Per employee per year |
Ages 16 to 49 | $90 | $1,080 |
Ages 50 to 64 | $100 | $1,200 |
Ages 65 and up | $110 | $1,320 |
Additional family member (age 3+) | $50 | $600 |
Put that against the numbers above. A year of unlimited primary care for a 35-year-old employee costs $1,080, roughly 12% of the average small-firm single premium, and less than half the $2,631 deductible they would have to clear before insurance paid for an office visit.
Futhermore, our small business partners have access to even lower pricing than outlined above - we truly believe in making it as affordable as possible for you to run your business and take care of your people. We will discuss your low-cost membership rates during your onboarding call.
What the Membership Actually Covers
It is unlimited primary care, and for most small teams it covers nearly everything that would otherwise send someone to urgent care.
• More than 28 included services, from annual physicals to chronic condition management, women's and men's health, and mental health care
• Same-day or next-day sick visits, plus direct phone and text access to the care team
• Our pharmacy program, launched in January 2026, which home-delivers over 1,000 maintenance medications and covers more than 95% of the most prescribed generics in the country
• Labs at cost, with a CBC at $6.50 against roughly $32 at a standard practice
• 30 to 60 minute appointments with Kelly Botta, PA-C or Colleen Shendow, PA-C
No copays. No deductible. No claims to file.
Three Low-Cost Ways to Set It Up
Most of the small businesses we work with use one of these three.
1. As a standalone benefit, if you offer nothing today. If you are in the 41% of small firms without health benefits, this is the cheapest way to start offering something real. A membership per employee is a fraction of a group plan premium, it takes no underwriting, and there is no renewal cycle. Your team gets a provider they can text tomorrow.
2. Paired with a higher-deductible plan. Keep catastrophic coverage for hospitalization and surgery, move to a plan with a lower premium, and put part of what you save into memberships. Your employees end up with better everyday access than they had on the richer plan. Over 75% of our members carry traditional insurance alongside their membership.
3. Employee-paid, with HSA dollars. Since January 1, 2026, employees can pay direct primary care fees from a health savings account, up to $150 a month. Every one of our individual rates fits under that cap. You set up group access, your employees enroll with pre-tax money, and your benefits budget does not move at all.
What a Winchester Business Owner Says
"Smartypants Medicine has made it easy for myself and my employees to get any care that they need while keeping workflow in motion."

That last part is what small employers feel first. When a crew member can text a provider and be seen the same day, you are not losing them for an afternoon in an urgent care waiting room.
Is It Worth It for a Business Your Size?
Is there a minimum group size? No. We work with employers from a handful of people up.
Does this replace our health insurance? No. Direct primary care is not insurance, and Virginia law is explicit about that. It covers primary care only, so your team still needs a plan for hospitalization, surgery, imaging, and specialists.
What if our costs are driven by a few big claims? Then primary care is not your lever, and we will say so. A membership works on everyday utilization: sick visits, chronic conditions, prescriptions, labs. If one catastrophic claim is driving your renewal, we would rather tell you that in the first conversation than sell you something that will not help.
How fast can we start? New members are typically established within about two weeks. More answers are on our FAQ page.
